			<content:encoded><![CDATA[<p>A weakened immune system and unhealthy lifestyle habits such as smoking put HIV-infected patients at increased risk for cancer, according to a new study. The researchers also said that starting antiretroviral therapy at an earlier stage of HIV infection might reduce cancer risk.</p>
<p>The primary goal of the study, one of the first to compare the risk of cancer in HIV-infected patients, was to determine how much of the increased risk was the result of the disease and how much was due to other risk factors, such as smoking, the researchers said. The study authors compared the rates of 10 types of cancer that occurred among HIV-infected patients and HIV-free patients from Kaiser Permanente in Northern and Southern California between 1996 and 2008.<br />

Six of the cancers were more common in HIV patients than in HIV-free patients, including Kaposi&#8217;s sarcoma, non-Hodgkin&#8217;s lymphoma, Hodgkin&#8217;s lymphoma, melanoma, anal cancer and liver cancer. Lung and oral-cavity cancers were also more common among HIV patients, but most of the risk of those cancers appeared to be associated with lifestyle habits such as smoking.</p>
<p>Prostate cancer was less common in HIV patients than in HIV-free patients. Further investigation suggested that a weakened immune system was associated with the increased risk of cancer in HIV patients.</p>
<p>&#8220;Taken together, we believe our results support cancer-prevention strategies that combine routine prevention activities, such as smoking cessation, with earlier HIV treatment to help maintain a patient&#8217;s immune system,&#8221; lead author Michael Silverberg, a research scientist with the Kaiser Permanente Division of Research in Oakland, said in a Kaiser news release.</p>
<p>The study appears in the current issue of the journal Cancer Epidemiology, Biomarkers and Prevention.</p>

The latest proposals from the European Commission amend those made in 2008, following criticism that the original ideas went too far in loosening restrictions governing communications between pharmaceuticals companies and patients.</p>
<p>John Dalli, European commissioner for health and consumer policy, said in a statement the new proposals would &#8220;further strengthen the control of authorized medicines.&#8221; The new proposals from the Commission, the executive arm of the European Union, would allow information in only certain areas, such as information on the label and on packaging leaflets, information on prices and clinical trials, and instructions for use.<br />

An earlier idea of letting drugmakers publish information about medicines on websites or in print &#8212; for example, in health supplements in newspapers &#8212; has been rejected under the new proposals, which state: &#8220;A publication in general print media will not be permitted.&#8221; The tough line may disappoint drugmakers wanting more leeway to provide information directly to consumers in Europe, which they argue is needed in part as a counterbalance to sometimes unreliable data provided on the Internet.</p>
<p>But the Commission&#8217;s new line was welcomed by the European Public Health Alliance (EPHA), which represents healthcare professionals and patients, for keeping advertising at bay. &#8220;The previous proposal was just a disguised way of giving pharmaceutical companies enough flexibility to promote their products directly to the public, in order to boost the sector&#8217;s growth,&#8221; said Monika Kosinska, the group&#8217;s secretary general.</p>
<p>In fact, the European pharmaceutical industry had never asked for a green light for U.S.-style direct to consumer (DTC) advertising, which some company executives anyway now view as a costly mistake. DTC advertising of prescription drugs is only permitted in the United States and New Zealand, and the practice has been widely attacked by U.S. consumer groups, especially in the wake of the 2004 withdrawal of Merck &#038; Co&#8217;s heavily promoted painkiller Vioxx.</p>
<p>Instead, drugmakers in Europe would like to steer a middle course between full-on television adverts and zero communication. &#8220;Those citizens seeking information on their disease or therapy should be able to access it in both user-friendly formats and in their own language,&#8221; the European Federation of Pharmaceutical Industries and Associations said on Tuesday.</p>
<p>Consumer groups have long resisted any loosening of restrictions, arguing that drugmakers cannot be trusted to provide unbiased information. The European Consumers&#8217; Organization BEUC said the redrafting of the plans were a first tangible effect of the decision to move competence on pharmaceuticals policy from the Commission&#8217;s industry division to the directorate responsible for health.</p>

			<content:encoded><![CDATA[<p>An experimental malaria vaccine tested on children in Burkina Faso has shown &#8220;a high level of efficacy&#8221; in protecting against the disease, a study published in the United States said Wednesday. The study was initially planned to study the safety and immune response of the vaccine, known by the name MSP3.</p>
<p>&#8220;However, as malaria attacks were documented as part of the safety follow-up, the investigators decided to explore the protective effect of the vaccine,&#8221; said report appearing in Thursday&#8217;s New England Journal of Medicine. &#8220;Results indicate a high level of efficacy.&#8221; In the study, 45 children aged 12-24 months were randomized into three groups receiving doses of either 15 or 30 micrograms of the experimental malaria vaccine, or the control vaccine against Hepatitis B.&#8221;Comparing the groups, they found a striking difference,&#8221; the study said.<br />

It found children who received the new vaccine at either dose had incidence rates three to four times lower than children who did not, &#8220;yielding efficacy rates of 64 and 77 percent protection against clinical malaria,&#8221; the journal article said.</p>
<p>&#8220;I found the results of this preliminary study in Burkina Faso to be most encouraging,&#8221; said Louis Miller, former chief of the Malaria Vaccine Development Branch of the US National Institutes of Health, in the journal. &#8220;Larger efficacy trials in diverse epidemiological settings will be required to confirm these results.&#8221;</p>
<p>The study was led by scientists from the National Center for Research and Training on Malaria in Burkina Faso, the London School of Tropical Medicine and Hygiene and the Paris-based Pasteur Institute. The researchers found those receiving the vaccine at either dose had more antibodies protecting against malaria. &#8220;Despite the limitations of the design and small sample size, these results strongly suggest a significant protective effect over the follow-up period,&#8221; the study said. Malaria claimed 781,000 lives in 2009, according to the UN&#8217;s World Health Organization (WHO). About 90 percent of malaria deaths each year occur in Africa and 92 percent of those are children aged under five.</p>
<p>Scientists say fighting the disease is complicated because the parasite, Plasmodium falciparum, is several hundred times larger than viruses like measles or polio, but is also as variable as viruses like influenza and HIV. &#8220;In addition to demonstrating safety, the vaccine induced the desired type of immune responses in all vaccinated children: those associated with natural protection observed in adults,&#8221; the study concluded.</p>
<p>&#8220;This provides hope that the above approach may provide an artificial way to shortcut the 10-20 years of malaria exposure needed to acquire that highly desirable immune state seen in adults.&#8221; The only malaria vaccine in advanced clinical trials is the RTS,S vaccine being developed by British drugmaker GlaxoSmithKline, which could become available in 2015. A 2008 study showed the RTS,S vaccine with an efficacy of 53 percent for young children and 65 percent for infants.</p>

			<content:encoded><![CDATA[<p>Keeping your doctor up-to-date on cancers in your family could put you on the fast track to screening tests, according to a study that takes a new stab at personalizing preventive medicine. Researchers found that from age 30 to age 50, the number of people who would be candidates for early colon cancer screening based on family history jumped from two to seven percent.</p>
<p>For breast cancer, the number of women qualifying for earlier and more sensitive screening went up from seven to 11 percent over the same period. &#8220;Doctors should take a family history every five to 10 years,&#8221; said Dr. Sharon Plon from Baylor College of Medicine in Houston, who worked on the study. &#8220;Only taking a good family history the first time you see someone means you will miss important changes.&#8221;<br />

Having a close relative with cancer often puts people at increased risk of developing the disease themselves. That&#8217;s why groups such as the American Cancer Society (ACS) have tailored their screening advice according to people&#8217;s family histories. For instance, the ACS recommends annual magnetic resonance imaging (MRI) in addition to the less-sensitive mammograms for women between 35 and 60 if they have a first-degree relative diagnosed with breast cancer before age 30.</p>
<p>But those recommendations are not universally accepted, and some researchers say there is little conclusive knowledge about the relative harms and benefits from more-intensive screening. Widely acknowledged downsides of screening include false alarms that lead to painful, expensive diagnostic procedures, as well as detection of relatively harmless pre-cancerous cells.</p>
<p>&#8220;The more we go after these tiny cancers, the more we increase the risk of overdiagnosis,&#8221; said Dr. Michael LeFevre of the U.S. Preventive Services Task Force (USPSTF), who was not involved in the new work. Overdiagnosis refers to those tumors that would never have bothered people in the first place &#8212; meaning that treatment will do more harm than good.</p>
<p>Still, LeFevre said a person&#8217;s family history of cancer is an important thing to consider when making screening decisions. &#8220;I certainly think it&#8217;s appropriate to update the family history periodically,&#8221; he told Reuters Health. &#8220;This article suggests that family history is a moving target.&#8221;</p>
<p>The study, published in the Journal of the American Medical Association, is the first to test how new cancers in the family influence a person&#8217;s candidacy for earlier or more-intensive screening.</p>
<p>The researchers used national data from the Cancer Genetics Network, including the family histories for colon, breast and prostate cancer of some 11,000 people. Looking back at the family histories, the team found the number of candidates for extra screening went up as more cases of cancer surfaced among their relatives.</p>
<p>The jump from two to seven percent for colon cancer, for instance, means that over two decades an additional five out of 100 people would have become early screening candidates if they&#8217;d kept their doctors up-to-speed on new cancers in their family. For breast cancer, an additional four in 100 women would have become early-screening candidates, and for prostate cancer, an additional one in 100 men.</p>
<p>&#8220;What we found is that if you take a good family history at age 30, you will only pick up part of the important data and that people&#8217;s family history of cancer continues to change until they are 50,&#8221; said Plon, adding that doctors usually do a poor job of taking patients&#8217; family histories. Importantly, however, the study did not test whether screening those new candidates would actually save lives.</p>
<p>An editorial published alongside the new results highlighted questions about the risks and cost of screening. &#8220;It is plausible but still unknown whether family history increases the likelihood that breast cancers, prostate cancers, or colon adenomas found by screening are clinically significant,&#8221; wrote Dr. Louise Acheson, of Case Western Reserve University School of Medicine in Cleveland.</p>
<p>For instance, she said, adding annual MRI exams to mammograms would increase the cost about 10 times. On the other hand, it&#8217;s possible that low-risk women in their 40s might choose to forgo screening, Acheson added. &#8220;The benefits, harms, and empirical results of such an approach are ripe for investigation,&#8221; she noted.</p>
